CD4 T cell differentiation is crucial for balancing effective immune defense and disease pathology. 

Stem-like CD4 T cells are present among antigen-specific CD4 T cells. This unique population retains self renewal and differentiation capacity, and helps to sustain effector responses that mediate disease outcomes across chronic conditions.

Stem-like CD4 T cells may also exhibit multipotency to differentiate into various helper subsets, and thus provide a potential explanation for CD4 T cell plasticity.

Targeting stem-like CD4 T cell differentiation could represent a novel therapeutic strategy for immune modulation across various diseases.
