IL-6 can support repair or promote inflammation in the CNS, depending on how and when receptor signalling is engaged. 

Classical IL-6 signalling is relatively restricted, whereas trans-signalling expands IL-6 responsiveness to gp130-expressing cells.

Evidence from neurodegenerative and neuroinflammatory disorders suggests that trans-signalling capacity may be more informative than IL-6 levels alone.

IL-6 trans-signalling can amplify glial, neuronal, vascular, and immune-cell interactions that contribute to chronic neuroinflammation.

Selective inhibition of IL-6 trans-signalling may offer a way to limit inflammatory pathology while preserving beneficial classical IL-6 functions.
